For Beneficiary:

  1. Original & photocopy of National Identity Card (CNIC), Pension Book & Gratuity Certificate.
  2. For children Less than 18 years, original and photocopy of B-Form
  3. For more than 18 years (Female), original CNIC card and Un-married certificate which must be verified from Secretary Union Council, Member Cantonment Board or Secretary Soldier Board.
  4. Widows, who are receiving pension, bring original and photocopy of CNIC and Pension book.

For CNE/Private Patients:

  1. CNIC
  2. If any Lab reports and medical prescriptions advised by consultant.

For Panel:

  1. Referral Letter
  2. Must mention the required OPD in the Referral Letter
  3. Must be sign & stamp by the respective department of their organization.

For Beneficiary:

    • Winters: 8:00 to 14:30

Personals of Mujahid Force, who are retired after 12 oct 2012, can avail treatment of themselves but their families are not entitled.
